Faith Based Counseling Upon Request
Insurance and Payment Information
Rapha Counseling is dedicated to providing high-quality counseling services to our clients. We collaborate with a range of insurance companies to facilitate coverage for our services. Please note that while insurance coverage is an option, it is important to be aware of the terms and conditions of your specific plan. The coverage is subject to contract limits and the member's status on the date of service. Any accumulated amounts such as deductibles may change as additional claims are processed.
We kindly remind all our patients and clients to stay informed about their copay amount and annual deductible balance. It is the responsibility of the patient/client to settle outstanding balances on their account promptly, in accordance with their insurance contract.
While most sessions that take place in our offices are typically eligible for insurance reimbursement, certain services such as telephone consultation, email communication, document review, and report writing may not be covered by insurance. In such cases, a fee will be assessed for these services.
Accepted Insurances
At Rapha Counseling, we receive benefit information from your insurance company, which represents estimates only. Please note that we are not liable for co-pays and/or deductibles that may differ from the information provided when verifying benefits. By receiving services at Rapha Counseling, the client/guarantor gives permission for us to contact any third-party payer for payment. We will submit claims to the insurance company on behalf of the client/guarantor.
We currently accept a wide range of insurance plans, including but not limited to Aetna, Anthem, Cigna, Humana, United Health Care, and more. For the latest information on accepted insurances, please reach out to our team.
Additionally, we work with various Employee Assistance Programs (EAP) including Aetna, Anthem, Carelon Behavioral Health, Corporate Counseling Associates (CCA), Health Advocate EAP, CuraLinc Healthcare, and Mutual of Omaha EAP.
