Insurance Information
Statement from all insurance companies to patients
Unless otherwise required by state law, this notice is not a guarantee of payment. Benefits are subject to all contract limits and the member's status on the date of service. Accumulated amounts such as deductibles may change as additional claims are processed.
Please Note
Per EVERY insurance agreement, it is the responsibility of the PATIENT/CLIENT to know their copay amount and annual deductible balance and to pay your outstanding balances on your account promptly per your contract.
Sessions that occur in our offices are eligible for insurance reimbursement. However, other services such as telephone consultation, email communication, document review, and report writing may not be covered by insurance. A fee will be assessed for these services.
Accepted Insurances
Benefit Information is given to Rapha Counseling from your insurance company and represents estimates only. Rapha Counseling is not responsible for co-pays and/or deductibles that may differ from what Rapha Counseling is told when verifying benefits. The Client/Guarantor gives permission to Rapha to contact any third-party payer for payment. Rapha Counseling will submit claims to my insurance company on behalf of the Client/Guarantor.
